You will need for first visit:
Your first visit to BusyBodies will consist of an evaluation of symptoms and history, caregiver education in an initial home program and an observation of how your child moves in the clinic environment. Please bring your child dressed comfortably and in shorts, if seasonably possible.
A Referral
A referral from your pediatrician/ doctor is needed if you plan to file on your insurance.
This referral should be faxed from the physician’s office to 877-798-4959.
An evaluation can be completed without a referral if you plan to pay out of pocket for your first visit
